Fill out all necessary information as clearly as possible. Applications that are not legible or missing information will be returned. The state and district form has been updated. PLEASE PRINT CLEARLY.

For state team, you must enter as an equal number of boaters and riders. If your team needs more than one application use more, but send the team apps in together as one team.

The first day applications will be accepted and post marked is February 14th, Valentines Day.
